Slipped Disc or Disc Bulge by Alex Chiropractor Guildford

I often hear the phrase slipped disc from patients who come into The Guildford Spine Centre – Chiropractor Guildford. The phrase slipped disc is very confusing and I do my best to explain the reasons it is wrong or more misleading than it should be.

The Spinal cord runs from the base of the brain in the skull right down to the tips of your fingers and toes. It runs inside the spinal canal in the spine itself. Along the way and between every vertebrae part of the nerve branches off and fees out into a certain part of the body. Among other things the nerve supplies the muscles nearby telling them when to contract, it also senses pain and light pressure and allows the body to tell exactly what position it is in. That’s why when you turn the lights out and you can’t see yourself you still know where your arms and legs are.

The disc in the spine sits in between every two vertebrae and acts as a shock absorber and also allows all the full and free movement you can do with your spine. Instead of the disc ‘slipping’ out of position like a brick in a stack of Jenga it actually tears and bulges. If this tear and bulge is large enough it can press on one of the nearby nerve roots and cause leg or arm pain. This is referred to as Sciatica.

That means when you treat a disc bulge it is not simply a case of pushing it back into position. Sadly the body does not work like that. THE TREATMENT OF BACK PAIN

Leg Pain While Walking – Guildford Chiropractic

I have had a number of interesting patients this week that were seeking help at The Guildford Spine Centre – Guildford Chiropractic. Two cases presented with the same issue. Both with unusual symptoms of neurogenic claudication causing leg and back pain. This both leg pain while walking and also in one case severe back pain for the patients.  Lumbar spinal stenosis is the most common reason for spine surgery among individuals older than 65 years (Deyo 2010). Over the next 20 years the incidence of new cases of neurogenic claudication is expected to rise dramatically. This will be when an estimated 23% to 25% of the population will be older than 65 years (Government of Canada 2006).

Neurogenic claudication  is the medical term to describe pain that comes on while walking. Pain can often be dispersed over a variety of poorly defined areas in the lower back and legs. One patient the pain experienced was in the lower back. Experiencing the sensation of ‘heavy legs’ while walking.

Causes

Damage to the nerves in the spine causes Neurogenic claudication. Claudication occurs where the spinal cord and the nerves that exit at each level known as the nerve roots. The pressure on the nerves is caused by narrowing of the spinal canal called spinal stenosis. A number of diffrent factors will cause this; osteoarthritis (wear and tear) on the spine can cause pressure on the nerve. A slipped disc (disc protrusion) that pushes on the nerves can also cause this. Certain people have a narrow spinal canal which can leave them more prone to these issues. Patients are commonly ages over 60 with these problems. The ligaments of the spine can calcify and thicken with age as well as the bones and joints.

Diagnosis

Patients with neurogenic claudication will experience leg pain while walking will occur but if they bend forwards from the lower back this eases their pain. The pressure on the nerves is eased in this position. Sitting can also ease the pain as again this stretches the spine opening up the spaces between the vertebrae. There are two types of claudication and neurogenic with nerve compression and vascular. Where narrowing of the arteries causes a reduced blood flow. I won’t go into too much anatomical detail of vascular claudication as that is a subject on its own. However a bike test is the test that would distinguish between the two causes.

A Chiropractor will conduct a physical examination, full case history and possible further imaging tests. This may include an MRI or CT scan or vascular testing.

Treatment

Our Chiropractic treatment is dependent on the person and symptoms. Initially treatment is conservative such as Chiropractic care. At The Guildford Spine Centre this involves relieving muscular tension in the surrounding soft tissue. Mobilising the lumbar spine and relieving pressure and inflammation on the lumbar spinal nerves. This can be through a variety of mobilisation and manipulative techniques.

If Chiropractic treatment is ineffective steroid injections or surgery may be the next step. However our Chiropractor in Guildford will do all he can to prevent the need for this.

A study showed that with 3-4 weeks of Chiropractic treatment headaches can be cut by half. One high quality study showed Chiropractic treatment to be superior to deep massage, placebo and no treatment.

The Guildford Chiropractor Alex Symington at The Guildford Spine Centre

I would like to take this moment on a Monday morning to introduce myself. I am the new Guildford Chiropractor, working in the Guildford Spine Centre.

My history to date involved 4 years practice in London, where I have opened and established a Clinic in north London serving the local population, forging good links with the local community and treating patients with a wide variety of ailments.

I have now moved to the beautiful county of Surrey where I now live and work. Being a Chiropractor in Guildford is both a big challenge to create the reputation that has taken many years to establish in London is daunting but exciting at the same time. The Spine Centre Guildford – Chiropractic Clinic is a cumulation of many years work along with a bit of luck and some great support along the way.

Whilst deciding on where would be the best location to both live and practice as a Chiropractor I explored many options but none had so many positives as becoming a Chiropractor in Guildford. The town is ideally located both close to London but in the countryside to have the rural feel. The population is healthy and aware of how Chiropractic can benefit the young through to the elderly and the positive impact it can have those that seek our care at The Guildford Spine Centre – Chiropractic Clinic.
